Important Reminder About Your Healthcare Flexible Spending Account Benefits Card
Debit cards for your spouse or dependent over the age of 18 can be ordered directly through our secure online portal, at no additional cost. To request a card, simply log in to your account and choose Benefit Card Order Form from the Quick Links menu. For security reasons, cards cannot be requested by phone.
